As our young people with special needs turn 16 years old, a critically important part of the educational planning process is transition planning. Transition planning involves assisting the student, with their family, in identifying their dreams and goals for the future after public school, and developing a plan to get there.
The plan may include courses and classes to be taken, life skills, career exploration, vocational experiences and linkages to agencies that serve adults with disabilities.
